Take your point about NZ taking a step forward but when and how did an offside line ever become active if no English player was engaged and a ruck had not been formed?  Even if it had been, why was the NZ forward at 8 not penalised for not binding correctly to it.

Every sympathy for the ref and his AR but none at all for the TMO who saw fit to make the call and then focussed only on a situation where he - like the English forwards before him - assumed that there was an active offside line - 2/10: could and should have done a lot better Angry

There is nothing wrong with your eyes. A ruck only need one person on their feet over the ball, not one from each side. The law changed last year after the England  v Italy fiasco
